Program Description: Great Oaks serves adults in southwest Ohio with disabilities who have a goal of working independently in the community. Referrals are received from other community rehabilitation agencies, families and private individuals. The following steps meet individualized attention and lead to success:
q Receive referral from community rehabilitation agencies, families and/or private individuals
q Link referral to all appropriate community agencies that will support the person in the employment process
q Meet individual and gather information relating to their interests, skills, abilities and other parameters of a job search
q Provide students with resume development, interviewing, and other work readiness skills
q Provide individualized job development services that match needs of the consumer with open positions
q Perform job analysis of prospective job to make best job match possible for employer and potential employee
q Coordinate job coaching, travel training, background checks and other supports necessary for consumer to begin employment
q Work with job coach, employer, employee, and family during the initial phases of employment to problem solve any issues
q Coordinate any adaptations and accommodations necessary for success on the job
q Maintain communication with all parties as consumer gains efficiency and effectiveness on the job
q Assist the job coach and employer in the fading process
q Facilitate follow along and career advancement services for consumer
q Provide the employer and employees with any specialized training in order to insure success for the employment site and consumer (i.e. ADA training)
Eligibility Criteria:
The consumer must:
q Have a documented disability or behavior which interferes with success in gaining and maintaining employment
q Be able and willing to access independent transportation
q Have a desire to work in the community
